Registration Information: Carefully read and complete in full the registration form and the parent/guardian agreement. Parents are encouraged to fill out both forms. Please print in blue or black ink. Do not forget to sign! Campers are not restricted to attending only one camp! Attend as many as you like. They are all different. Invite a friend! Grade entering in the following fall determines grade of camp to attend. Exceptions can be made. Call us! Make checks payable to “Lake View Camp.” Mail registration form and payment to: Lake View Camp, P.O. Box 836, Pella, IA 50219. Each camp is filled on a first-registration-received basis. Do not forget about the May 1 Early Bird Registration Date! Registrations must be postmarked by May 1 for the discount price. Please notify your church regarding the camp you plan to attend. Some churches offer financial or transportation assistance for campers. The health form must be filled out completely before your registration can be processed. Friend Discount: Invite a friend to come to camp with you and receive a $15 discount on your camp fee! The friend must come to the same camp as you and has never attended one of Lake View’s camps before. Please write “Friend Discount” on your form and have your friend write “Friend of __________” on their form. If possible, send the two forms together. The discount applies only to your camp fee (not your friend’s camp fee). Invite as many friends as you want but only one discount applies! The Friend Discount does not apply to the Work Trip, Grandparent/Grandchild Camp, or Angel Tree Service Camps. Mail: Parents and family members are encouraged to send mail to their camper. Letters from home are always fun! Remember to send it early to make sure it arrives while the camper is still at camp! Include the camper’s name on the front of the envelope. Camp Pine Lake 23008 W. This is an expensive waste for our campers (and their parents!). As a way to target the problem of lost belongings, we are encouraging campers to LABEL all belongings. If your camper does leave something at camp, call (641) 628-2160 to claim items. Items found will be held until September 1. Medication: All medications, prescriptions, and non-prescription drugs must be turned in to the camp nurse or camp staff at registration. All medications must be clearly labeled with camper’s name, medication name, dosage, frequency, and times to be taken. Medications will be distributed under supervision of the camp nurse or camp director as instructed by a parent/guardian or physician. Prescription medicine must be in pharmacy container. Scholarships: Partial camp scholarships are available upon request. We try to keep our camper fees as low as possible as we do not wish cost to be a reason not to attend camp. Please contact us about scholarships or payment options. Scholarships are available for only one camp per person. Scholarships are granted on the basis of economic need and generally do not exceed half of the camp fee. Telephone Calls: We discourage phone calls to or from camp because they can make homesickness worse. We will contact you if needed. In case of an emergency, call the director’s cell phone, (641) 780-3031, and leave a message. We will return your call as soon as possible. Camper cell phones are discouraged. They may be held by staff until the end of the week if determined to be a disruption or distraction. Canteen: Money can be turned in at registration for the child to purchase snacks throughout the week of camp. The remaining money will be returned to the child the last day of camp. Returns under $1.00 will not be made and will be given to the mission fund. Refunds: Refunds are given if requested at least seven days before the fist day of your camp. If you cancel less than seven days before your week of camp, refunds are given only for medical reasons or a family emergency. All refunds are subject to a $25.00 administrative fee. Fees may be transferred to an unregistered camper as a replacement if proper registration forms are completed and approved by the camp director.
